Timeline for DUI Ordered Treatment or DMV-only DWI Level II Treatment /4 plus
- Fill out all pre-intake documents then Complete your program enrollment and assessment with our DUI Therapist.
- If sentenced, submit your DUI sentencing order and your Alcohol Evaluation to frontdesk@recovery-systems.org.
- Make sure all program forms have been signed electronically via your secure online client portal.
- Submit your Affidavit of Enrollment form to the DMV, if required for license reinstatement (this form is provided by the front desk after attending your first class).
- Begin DUI Education group sessions and pay for your classes (sliding fee scale for Medicaid clients or low-income and sober living).
- Complete your DUI Education requirement (generally 12 weeks= 24 hours and 3 months required to complete and then move on to Therapy).
- Meet with the Therapist to complete treatment orientation to transfer into DUI Therapy classes of your choice (if required).
- Begin DUI Therapy group sessions and pay for your classes prior to attending.
- Complete your DUI Therapy requirement (21-43 weeks, depending on your assigned DUI treatment track- IOP/EOP ).
- Meet with the DWI therapist to sign your DUI Program discharge paperwork.
- Submit your final DRS report to the DMV and your Probation Officer (this form is provided by the Therapist/Front desk once your discharge is complete within 10 days of completion).

Telehealth sessions: Our enrollment process for new DUI clients is now fully automated and is completed online through our HIPPA-compliant virtual intake system. And Reliatrax.
The first step is to set up your file in Reliatrax by calling 719-578-5433
Then we send the pre-intake packet for You to complete all pre-intake documents online.
Then call the front desk for a ZOOM telehealth appointment or email us to meet for the review with a therapist in a telehealth session on Zoom to answer your questions.
Enrollment steps: 1. Create your secure Recovery Systems PC Reliatrax client portal (online) with the front desk at 719-578-5433
2. 50.00 intake fee unless vouchered or Medicaid eligible.
3. Complete the intake appointment with a therapist online via ZOOM;
Pick your class days and times and when you wish to begin attending. Provide all court documents required for treatment. Fill out a release of information for your PO/Casemenager to verify your attendance and give us UA/BAC results through the portal.
4. Sign service plan Portal. Education class comes first and no insurance ever pays for Education to this date (12-11-2022 – Must be private pay or Voucher from court or Parole)
4. Receive the book via the email/portal or walk in for a hard copy.
5. Throughout the process please provide all court documents to us through the portal in a timely fashion
A. Case number judicial district next court date
B. PO or case manager name and contact information, are required for all Releases of information and to verify Urine or Breath testing results.
C. ADES eval from Probation
D. Final sentencing order/deferred judgment – Court order –

6. Complete your DUI Education requirement (generally 12 weeks or makeup what you miss at the end)
7. Attend a DWI “orientation session” (approx. 1 hour), to Therapy with DWI Therapist. for treatment planning and confirmation of all necessary information: Case Number- judicial district- ROI- PO- court order – ades eval UA’s ETC.
($50.00) bring all court documents or send them through the portal.

8. Begin Monitored sobriety if not already begun with PO- send us results/ sign Release of information for Lab.. (if required, see Sentencing order)
9. Begin Therapy online via Telehealth ZOOM. (texted reminders should come 24 hours before each class.
10. Complete Therapy.
11. Confirm with the Probation Officer or therapist that all probation requirements have been met.
12. Come in to fill out Discharge papers and get DRS, and a memento for completion. Or contact Frontdesk@recovery-systems.org for online completion.
Level II always therapy and ed for all DMV clients
LEVEL II Ed+ 24 hours over 12 weeks (1, 2hour class weekly)
Then Therapy tracks (1, 2hour class weekly)
A = 42 hours 21 weeks
B= 52 hours 26 weeks – 6 months
C= 68 hours 34 weeks
D = 86 hours
4 plus = 180 hours over 18 months (no ed required)
EOP = one group and one individual weekly for 12 weeks 36 hours
IOP = 9 hours per week for 4-8 weeks PRN 36 hours
